In light of the Coronavirus (COVID-19) spread, we wanted to notify you
Our clinic is OPEN and we are seeing patients for ongoing medical care. In support of social distancing, we are also offering telemedicine visits to our patients to ensure that your healthcare remains uninterrupted. Currently, we are able to accommodate established patients within 1 to 3 days of your request to be seen. While social distancing and other measures to prevent infection are of utmost importance, it is equally important to remain healthy and keep your pre-existing medical conditions well controlled. You can do this by continuing regular follow-ups with your primary care doctor. Please call our office (801-519-2461) today to schedule your in-office or telemedicine appointment.

As a valued member of the Evolutionary Healthcare community, we appreciate the trust you place in us and want to inform you about how we are addressing the Coronavirus (COVID-19) situation. We are closely monitoring events in our local community and continuously updating our policies and protocols as a result of new information. Please know that our office is following all recommended guidance from public health authorities, including best practices for hygiene, infection control, and medical professional team health. We feel confident in our ability to continue seeing patients and providing primary care according to the quality of care you have come to expect and deserve. Our highest priority is to keep all of our patients and staff as safe as possible.

We wanted to notify you of the procedures we have developed to help keep you safe and healthy. We are working hard to help identify who may be at risk and administer appropriate testing.

For our healthy patients, we are running the clinic as normal. We do ask all patients to enter on the south side of the building (200 South). We continue to offer full services including physical; medication refills and diagnostic visits. All rooms and the waiting room are cleaned and sanitized throughout the day to maintain the safest environment for you and our staff.

If you are having upper respiratory symptoms consistent with COVID-19 (fever, cough, and shortness of breath) or you feel you have been exposed to COVID-19, we ask you to call the front desk at 801-519-2461 to schedule your appointment. When you arrive, please park by the west door. You will be asked to wait in your car until we are ready for you. You will be given a mask before entering the clinic. All doors will be opened for you as you enter the isolation room. At your visit, our staff and doctor will

assess your symptoms and provide you with appropriate diagnostic testing and/or treatment plan for your symptoms. Please be aware that not everyone brought into the isolation room will qualify for COVID testing; we are using the latest recommendations from the CDC and expert opinions in this evolving situation.

If you meet the criteria of COVID-19 testing we will call you with the results and the health department will also be notified. Anyone that has been tested is required to participate in self-isolation while awaiting results. We will provide you with a handout that outlines the rules and expectations of self-isolation. This requirement has been set up to protect your loved ones and the community around you. If you test positive, more detailed instructions will be given.
